Please read this Electronics Communications Disclosure thoroughly, as it contains information about your legal rights. This disclosure applies to all communications with me (the provider) via email or text from the contact information stated above, communications via contact forms on my website (www.newgrowthpt.com), communications through your patient account with Jane Software Inc, and any communications via other electronic means.
Electronic Communications Disclosure
By agreeing to this electronic communications disclosure, you agree to receive all categories of communication in electronic format only, unless formally requested in writing or required by applicable law. You understand that you must maintain an online patient account to access these communications. You may manage certain electronic communications preferences, such as frequency and method of appointment reminders, by accessing your online portal. You must agree to this electronics communications disclosure in order to use your online patient account, as use of the account requires some information to be transmitted online. By agreeing to this disclosure, you confirm that you have the hardware and software needed to access this information through your patient account. You have a right to receive written communications on paper, and may request to do so, even after agreeing to this disclosure, by contacting me at the contact information provided above. We reserve the right to deliver communications to you by paper if necessary, in which case communication will be mailed to you at the primary mailing address shown for you in our records, unless otherwise specified by applicable law.
By agreeing to this disclosure, you agree to receive the following information electronically:
Disclosures, notices and agreements related to your patient account
Disclosures pertaining to maintenance of your patient account
Any changes to terms or conditions of your patient account
This electronic communications disclosure, other privacy disclosures, and any disclosures or notices required by federal or state laws or regulations
Forms and documentation submitted electronically such as new patient intake forms, health questionnaires, and medical forms related to an upcoming medical appointment
Acknowledgment of policies that we request you to submit electronically
The minimum requirements to access electronic communications are as follows:
A personal computer with 128-bit encryption enabled and configured to accept cookies, internet access, a valid email account, and software to receive email messages. To retain communications you must have a printer connected to your personal computer or hard drive/storage space to download the communication. In certain circumstances, we may email you a communication and also post the communication to our website. You agree that by emailing you the communication and posting to our website, we have completed our responsibility of notifying you and that you have received the communication in a form that you can access and retain. You are responsible for notifying us of changes to your email address or other contact information, as well as updating your email address in your online patient account. If communications are sent to you at a deactivated, invalid, or incorrect email address or contact information that you failed to update, you agree that the communication was nevertheless made available to you if it was emailed to the email address or contact information listed in our records.
We will provide a paper copy to you of any electronic communication upon written request, up to 12 months after the communication was initially made electronically. Reasonable fees may be charged for printing and mailing of paper copies of communications.
You may withdraw your consent to this electronic communications disclosure at any time by providing written notice of your withdraw of consent by contacting me (your provider) at the contact information above.
Please print or otherwise save a copy of this disclosure for your records.
Mobile SMS Messaging Privacy Policy:
During intake we may collect information, such as name, phone number, and email address. We may use the information we collect to perform the services requested including billing, customer service, appointment reminders and other administrative requests. Mobile information collected will not be shared with third parties/affiliates for marketing/promotional purposes. This includes text messaging opt-in data and consent. All policies are followed as per CTIA guidelines 5.2.1. At any time if you want your information to be removed, you can contact us via our email address or regular mail, via the contact information listed above. Message frequency varies. Message and data rates may apply. For help, reply HELP or email us at newgrowthpt@proton.me. You can optout at any time by replying STOP to a mobile text/ SMS communication.
